Fonua-Blake received a sin bin penalty in the last 15 minutes of the Sharks' match against the Dragons, which enabled St George Illawarra's Setu Tu to score a late try, securing a 24-16 victory for his team.
If Fonua-Blake chooses to contest the charge at the judiciary and is found guilty, his suspension could extend to two matches.
